DSE Workstation Assessment

Use this form to request a workplace DSE (Display Screen Equipment) assessment. It may be completed by an employer, referrer, or the employee themselves – so long as all referral details are included. Our ergonomic specialists will assess the workstation setup and provide tailored recommendations. Once submitted, we’ll confirm receipt with you, confirm your details with the referrer and/or line manager, and coordinate the booking with you directly.
