DSA / SAAS Student Ergonomic Assessment Referral

This referral form is for use by assessment centres referring students for ergonomic assessments under the DSA or SAAS scheme. Assessments are available face-to-face or remotely (via MS Teams). Our trained assessors will review the student’s workstation, identify musculoskeletal risks, and provide tailored recommendations.

Once submitted, we will confirm the referral and coordinate directly with the student and/or referrer to book the assessment.
